How to Use the Digital Evaluation Parent, Teacher, and Health (DEPTH) Forms

Overview

The Digital Evaluation Parent, Teacher, and Health (DEPTH) Forms allow you to securely gather background information for a speech-language evaluation—without printing, hand-delivering, or chasing down paperwork.


In this tutorial, you’ll learn how to: 

  • Send digital health, teacher, and parent questionnaires
  • Collect responses via email or text
  • Support bilingual families with translated forms
  • Track completion and view responses in one place
  • Generate language exposure and use summaries automatically

Access DEPTH Forms

  • Open your DEPTH Forms Dashboard
  • Click “Add Form”
  • Select a client from your list or add a new one.
  • You’ll see three types of questionnaires available: 
    • Parent Questionnaires
    • Teacher Questionnaires
    • Health Questionnaires

Each form is designed to collect relevant information to support your evaluation.


Let’s start with the Health Questionnaire

  • Click Gather Health Information
  • Select the information you need:
    • Vision
    • Hearing
    • Other relevant health details
  • Click Continue
  • Enter the healthcare professional’s name and email
  • Click Continue
  • Preview the email and send the form

📱 Tip: You can also copy the form link and text it directly to the nurse or healthcare provider.


  • Copy the link
  • Paste it in a browser
  • View the form

What the Health Form Collects

  • Screening dates
  • Type of screening equipment
  • Examiner name and title
  • Results for vision and hearing
  • Additional medical or medication notes (especially helpful for complex cases)

Once submitted, responses are immediately viewable and ready to incorporate into your report.


  • Go back to the Forms
  • Hit refresh
  • You’ll see all of the information provided


Teacher Questionnaires

Teacher forms are built from the best intake questions across 50+ school districts, refined to focus only on communication-related concerns.

  • Select Teacher Questionnaire
  • Choose Select All (recommended for most cases) or customize questions for EI, clinic, or life skills settings
  • Add one or more teachers (ideal for middle school, bilingual programs, or multiple service providers)
  • Add the email address
  • Preview and send the form.

📱 Teachers can complete the form easily on their phone or computer.


We’ll complete the forms here so you can see what we’re asking the teachers to do.

  • Answer these questions about your student.

You’ll see that if you have a student who speaks a language other than English, you’ll use sliders to estimate the percentage of language exposure and use in each of their languages.


  • Use the sliders for an imaginary child.
  • Hit submit.
  • Click update.
  • Click view.
  • Click print or copy to incorporate them into your report.

Try it on a phone.

  • Go to Timothy Teacher
  • Click Copy Link
  • Paste it into a browser to see the questionnaire in mobile form. 

This is what your teachers will see.



Parent Questionnaires (with Translation Support)

  • Select Parent Questionnaire
  • Choose your questions or select all then click Continue
  • Add one or more caregivers (e.g., mom, dad, guardian)
  • Preview the form and email before sending

🌍 Language Support - Forms can be completed in 16 different languages- Parents can switch languages themselves, even if the email is sent in English - Responses return with:


  • The original language
  • English version of the question
  • Parent’s exact response
  • A general English translation for review or collaboration

Track Completion & View Responses

From the Child’s Forms Dashboard, you can:

  • See which forms are completed or still pending
  • Follow up with parents or teachers who haven’t responded
  • Open, copy, or print responses directly into your evaluation report

Your main Forms Dashboard also shows: 

  • All children 
  • In-progress vs. completed forms 
  • Total forms sent

  • View Referral Documents 
  • Download printable versions of all DEPTH forms

Bilingual Language Exposure Data

Remember the sliders in the parent and teacher forms? Let’s take a look at how we pull that together to get a measure of overall language exposure and use.

  • Back on the dashboard, click on Language Proficiency
  • Use the parent and teacher sliders to estimate: 
    • How much English the child hears across settings (school, home, weekends, evenings)
    • How much English the child uses across settings (school, home, weekends, evenings)

The inputs are automatically calculated—no manual calculations needed.


📊 Language Proficiency Report (Automatic)


Using the slider data from parents and teachers: 

  • Go to Language Proficiency from the dashboard 
  • View combined language exposure and use 
  • Generate a research-based report that includes:
    • Overall percentages
    • Parent vs. teacher input
    • Spoken vs. heard language
  • Click View Report and get a clear table with research cited on why this information is so important and how we use it. 

This report is especially helpful for bilingual evaluations and eligibility decisions.
